Abstract
We describe four adults in whom intestinal tuberculosis presented as a mass lesion clinically and radiologically indistinguishable from colonic or ileocaecal malignancy. All four underwent cross-sectional imaging reported as suspicious for neoplasm; three proceeded to surgical resection before a tissue diagnosis was available. Histopathology in every case showed caseating granulomata without dysplasia, and mycobacterial culture or nucleic acid amplification was confirmatory in three. All patients recovered fully on antitubercular therapy. In settings where tuberculosis is endemic, the pseudo-tumoral pattern should be explicitly considered before resection, and colonoscopic or image-guided biopsy with mycobacterial testing should be routine in any ileocaecal mass.
